GREETINGS TO ANIMAL SCIENCE FAMILIES

In this section you will find the Animal Science approval forms. If you have forms from previous years, recycle or throw them away as they are no longer valid. If you plan on showing, these forms are required at 4-H shows. For families new to 4-H, please call and ask any questions you may have.

On your APPROVAL FORM list all 4-H project animals to be shown. (Hint: it is usually best to include all eligible project animals...your favorite might not be feeling well on show day.) PRINT IN PEN. Complete the form including your mailing address with zip code and your age as of 1/1 of the current year. List one animal per line. Do not skip any lines. Print clearly. REMEMBER, ALL ANIMALS IN A 4-H PROJECT MUST BE SOLELY OWNED OR LEASED BY THE 4-H MEMBER. (No farm names as owner)

In order for you to show at the Fairs in 4-H classes, ALL ANIMALS MUST BE SUBMITTED FOR APPROVAL BY THE FOLLOWING DATES:

Deadlines for submitting ANIMAL APPROVAL FORMS to your County Extension Educator, 4-H for signature:

February 1: BEEF steers that will show at the Eastern State Exposition 4-H Beef Show

May 1: BEEF HEIFERS that will show at ESE, (excluding ESE steers), WORKING STEERS, DOGS, DAIRY CATTLE, LLAMAS.

June 15: ALL SHEEP, GOATS, SWINE, SMALL ANIMALS. POULTRY AND OTHER BEEF (not showing at ESE).

All 4-H project animals must be at least four months by show day to be shown, exhibited or brought to any given 4-H event. For instance, a lamb born on May 31, 2009 could not be exhibited or brought to a 4-H event before September 31, 2009.

REMEMBER...ALL ANIMALS MUST BE VACCINATED FOR RABIES 30 DAYS PRIOR TO FIRST FAIR!

SOME FAIRS are requiring veterinarian administered rabies shots this year, so plan accordingly. Some fairs may host clinics. Check fair entry information.

Please read the directions on the back of the approval form before completing in ink.

Completing forms in ink assures readable photocopies. Be sure to complete all columns! It will be returned to you if we do not have all the information and it is your responsibility to have the form complete prior to all deadlines: see deadlines enclosed. Your leaders can help with your questions, or give the office a call. Feel free to photocopy any of these forms. Approval Forms are now on-line. Check the above Merrimack County website and look for Animal Science Resources.

After filling out the form, remember to sign your approval form and have your parent or guardian, sign too! Your leader must review and sign the approval forms! Independent members must have your supervisor’s signature on the leader’s line on the approval front page. If you are in a club, the signature MUST BE YOUR ORGANIZATION LEADER’S, not your parents!!!!!

The 4-H Code of Conduct, and the Health Forms are a part of the approval form. Be sure to complete both sides and have all signatures on them. When going to 4-H shows, remember they are part of the information that will need to accompany you to the fair. If your medical health changes, remember to update the form. After all forms are complete, send them to the Merrimack County 4-H office, 315 Daniel Webster Highway, Boscawen, NH 03303. We will make a copy and return the signed original to you. You must bring this to all 4-H shows that you participate in.

The Hopkinton State Fair and other fairs will be asking that a copy of your approval form accompany your fair entry form.

If you do not have your own animal and are planning to, or have been leasing an animal, please request the Suggestions for Setting Up 4-H Leases which states the policies as well as guidelines for leases. (Lease guidelines are available from the 4-H office - call Lynn to request.) Completed Record of 4-H Lease must also come into the 4-H office at the same time as your approval form and also be complete. We have animal project record forms which will help you track your 4-H project. If you would like these, please call Lynn and request them.

Please be sure ALL INFORMATION is complete on your approval form, Code of Conduct & Health form, including signatures...before you mail it to the 4-H office...Double check now before mailing it!!!


REMEMBER...In order for your club leader and me to sign your approval form, you MUST be a 4-H member in good standing. Enrollment form must be in for the current year and you must meet the guidelines of 4-H and your club - attend meetings regularly, set goals, complete your project requirements and take part in community service.
