GREETINGS TO OUR HORSE FAMILIES

Here you will find horse approval forms. If you have forms from previous years, recycle or throw them away as they are no longer valid. If you plan on showing, these forms are required at 4-H shows. For families new to 4-H, please call and ask any questions you may have.

Please read the directions on the back of the approval form before completing the form in ink. Completing forms in ink assures readable photocopies. Be sure to complete all columns! Horses must be described or enclose a picture. It will be returned to you if we do not have the info and it’s your responsibility to have the form complete prior to all deadlines: April 1st for those aspiring for Eastern States and May 1 for all others. Your leaders can help with your questions, or give the office a call. Feel free to photocopy any of these forms. The 4-H forms can also be downloaded from add site here. There is also a form on the website that enable you to fill it out on line. You should list the division in which you will be showing your horse. This needs to be accurate and you will ride in this division all season. If unsure, ask your leader for help with this.

After filling out the form, remember to sign your approval form and have your parents or guardian sign the form and helmet release form too! Your leader must review and sign the approval forms. Independent members must have your supervisor’s signature on the leader’s line on the approval front page. If you are in a club, the signature MUST BE YOUR ORGANIZATION LEADER’S, not your parents!!

The 4-H Code of Conduct, and the Youth Health Forms are a part of the approval form. Be sure to complete both sides and have all signatures on them. If your medical health changes, remember to update the medical form with the 4-H office.

When your form is complete, mail it by the deadline to the Merrimack County 4-H office at 315 Daniel Webster Highway, Boscawen, NH 03303. We will copy your form and return the original for your records. You must bring this form with you to all 4-H shows you participate in this year. Keep in a safe place and do not lose them.

If you do not have your own animal and are planning to, or have been leasing an animal, please call the office to request the Suggestions for Setting Up 4-H Leases and a lease form or print it from the UNH Web site listed above. Completed Record of 4-H Lease must also come into the 4-H office at the same time as your approval form and also be complete.
