Virtual workshops at times that work for you


Living and coping with a chronic condition and/or chronic pain can be very challenging. We offer evidence-based self-management programs designed to give you the skills, tools and confidence you need to help take control of your chronic condition(s) and live life to the fullest. Caregivers and loved ones of those who manage chronic conditions or pain are also encouraged to participate.

Put life back into your life! This course is designed for anyone ready to learn the skills and tools to manage their chronic disease such as Arthritis, Asthma, Diabetes, Fibromyalgia, Hypertension, Multiple Sclerosis or other chronic health condition.

This program follows Stanford’s Chronic Disease Self-Management Program curricula.

Living Well with Diabetes

workshop series

Gain the confidence and real-life skills needed for living a full healthy life with diabetes!

If you or someone you care for has diabetes this workshop is for you!Learn techniques to deal with the symptoms of diabetes, such as fatigue, pain, hyper- and hypoglycemia, stress, and emotional problems such as depression, anger, fear, and frustration.

This program follows Stanford’s Diabetes Self-Management Program curricula.

What to Expect

Are you ready to put life back into your days?   

In these workshops, you will learn ways to manage symptoms and meet health goals.  Create a weekly action plan to help you meet your goals so that you can do the things in life YOU want to do!   

Gain the confidence and real-life skills needed for living a full, active and fulfilling life with chronic disease or pain. 

What is covered in these programs?  

  • Condition related problem solving 
  • Communication skills 
  • Managing emotions 
  • Physical Activity 
  • Strategies to deal with fatigue, pain, frustration, isolation and poor sleep 
  • Developing patient/physician partnership 
  • Use of community resources 
  • Nutrition 
  • Managing medications and how to evaluate new treatments  
  • Cognitive symptom management 
  • Benefit from the support and encouragement from others also living with chronic disease or pain  

More Information about these workshops 

  • This workshop series is free! 
  • 6 weekly interactive sessions
  • Each 2½ hour session builds on the previous week
  • Full participation in the 6 weeks is highly encouraged 
  • These workshops are live and are not recorded

These workshops are open to N.H. residents only at this time.   

These workshops are funded by a SAMHSA Rural Opioid Technical Assistant grant (H79TI083274) awarded to UNH Cooperative Extension.
